The Following Applications Are Available For Downloading: Please fill out the City of Fort Myers Application for Employment first. You will be contacted after the screening process if the Personal History Questionnaire is needed.

IMPORTANT: Additionally, The following documents must be provided with the completed applications. If the documents are not provided, your application will not be processed.

Applicant Checklist:

  • Birth certificate/Proof of U.S. Citizenship
  • Copy of Social Security card
  • Copy of Drivers License
  • Copy of Driving History from Any/All States in which you have possessed a drivers license
  • Copy of High school diploma or GED documentation.
  • Copy of Highest Educational Degree.
  • Proof of Legal Name Change (Adoption, Marriage, Divorce, etc)
  • Three Letters of Professional Reference (Someone known five years or more excluding family members) 
  • DD214/Military Discharge document.
  • State Exam Test Scores
  • Florida Law Enforcement Certification
  • Complete background investigation waiver (Form CJSTC 58) in Personal history questionnaire.
  • Complete affidavit of applicant (Form CJSTC 68) in Personal history questionnaire.
  • Any other documents reflecting your qualifications, if applicable

The applications along with the required documents should be brought or mailed to:
