Applicant Checklist:
- Birth certificate/Proof of U.S. Citizenship
- Copy of Social Security card
- Copy of Drivers License
- Copy of Driving History from Any/All States in which you have possessed a drivers license
- Copy of High school diploma or GED documentation.
- Copy of Highest Educational Degree.
- Proof of Legal Name Change (Adoption, Marriage, Divorce, etc)
- Three Letters of Professional Reference (Someone known five years or more excluding family members)
- DD214/Military Discharge document.
- Florida Law Enforcement Certification
- Any other documents reflecting your qualifications, if applicable
The personal history questionnaire along with the required documents should be brought or mailed to:
